The Nursing Department consists of three outpatient nurses and four PACT nurses that work closely with their teams and providers. Outpatient nurses work one on one with their providers and offer care coordination services to clients. They work closely with all of the service lines making sure that client’s needs are met in regards to medication management, insurance prior authorizations, laboratory orders and results, coordinating care with interagency and outside providers and services, and completing screenings and assessments with clients.

PACT team nurses work closely with their teams to serve a maximum of 70 clients all of whom have serious and disabling mental illness who are able to live in the community. PACT nurses meet client’s needs where they are through a team-based approach and are active and mobile in the community. They provide case management, medication management, coordination within their team and outside agencies, and also provide assessments and screenings to their clients.
